Forms and Documents

Written order forms are required for an appointment. If they are not received in advanced or on the day of the exam, the appointment may be delayed and/or cancelled.

Please inform the scheduling office of any special patient instructions when ordering a procedure.

If your office needs additional copies of the order form, please contact Angelina Atiles at (203) 276-2602.

Pre-Certification and Authorization Requirements

Many insurance carriers require a pre-certification or authorization number prior to the administration of a radiological exam. These insurance carriers often require the referring provider to provide the insurance company with some clinical information concerning your medical condition and, sometimes, prior treatments. Other carriers may simply require that the patient contact the carrier to obtain a pre-certification or authorization number.

Stamford Hospital requires that a pre-certification or authorization number be provided prior to the scheduled radiological exam.

Without your insurance carrier's pre-certification or authorization number you:

  1. May become fully financially responsible for any services rendered, OR
  2. Your appointment may need to be rescheduled

Please note that radiology services that most commonly require pre-certification or authorization numbers include, but are not limited to:

  • CT scans (all diagnostic exams)
  • MRI/MRA (all examinations)
  • Nuclear Cardiology

 Requirements

The following information, in addition to specific clinical information, may be required by the insurance carrier in order to approve a pre-certification or authorization number:

  • Patient Name
  • Patient Date of Birth
  • Tax Identification Number
  • Procedure
  • Diagnosis
  • Hospital address

It is strongly recommended that patients review their benefit information regarding pre-certification or authorization requirements prior to their appointments.

The hospital's Insurance Verification Team is available to assist with questions related to pre-certification or authorization numbers and can be reached at 203-276-7325.
