Our Social Workers assess patients for risk factors that would inhibit their safe, speedy recovery and discharge home.
Social workers at Stamford Hospital provide services to patients of all ages - from children to adults - who are admitted to our hospital. or who are seen on an out-patient basis. As part of your health care team, the professional social work staff helps patients and families understand and cope with medical, psychological, and social issues related to their illness.
Our Social Workers assess patients for risk factors that would inhibit their safe, speedy recovery and discharge home. Staff members are available to help you and your family with counseling, referral to community support groups and crisis intervention. They can help solve a range of non-medical problems that present obstacles to health and healing, including a poor community support system, domestic abuse, frailty, lack of financial resources, and poor judgment and decision-making. If you require transfer to another facility, they can help you to locate and plan the financing of an appropriate placement.
Following a personal assessment, our Social Workers gather available resources on the patient's behalf. Plans are made in conjunction with the patient and/or the patient's advocate to offer reasonable options and choices that are compatible with the patient's lifestyle.
Services provided include:
- Education and supportive counseling to patients and their families
- Coordination of patient transfers to a variety of specialized settings, including rehabilitative or skilled nursing facilities.
- Access to both public and private entitlement programs for inpatient and outpatient populations
- Support for patients and families managing chronic medical problems and crisis situations.
