Diabetes Self-Management Education Program*
At Stamford Hospital we know that good self-management for diabetes is the key to health, well-being and prevention of complications associated with uncontrolled blood glucose levels. Whether you are newly diagnosed or have been dealing with diabetes for many years, our Diabetes Self-Management Education Program* provides excellent educational services (group and individual) for people who have been diagnosed with diabetes -- Type 1, Type 2, or Gestational Diabetes.
Since we were established in 2001, we have obtained and maintained our American Diabetes Association (ADA) Education Program Recognition Certificate. The Certificate confirms that the Program meets National Standards, delivering high-quality education, an essential component of effective diabetes treatment. Our latest status will remain effective through 2015. All of our educators are Certified Diabetes Educators - nurses and dietitians who meet strict guidelines and are re-certified every five years through testing or continuing education credits in order to remain current with new developments in diabetes care.
For all patients, our Diabetes Self-Management Education Program* begins with a one-on-one assessment visit. Based on your individual needs, we provide the following types of education either individually or in a group setting, including core content as outlined by the National Standards for Diabetes Self Management Training.

Pre-diabetes and a Healthier Lifestyle
If you have been told you have pre-diabetes or even if you simply want to improve your lifestyle, Stamford Hospital offers individual education and counseling sessions with registered dietitians at both Tully Health Center and Stamford Hospital. At your first one-hour meeting, one of our expert dietitians will review your diet history and help devise an individualized eating and exercise plain tailored to your needs and preferences. Follow-up appointments may include a weigh-in.
