Stamford Hospital's Social Work Department is focused on supporting our medical services so that patients can focus on their recovery. Our Social Workers assess patients for risk factors that would inhibit their safe, speedy recovery and discharge home.
They can help solve a range of non-medical problems that present obstacles to health and healing, including a poor community support system, domestic abuse, frailty, lack of financial resources, and poor judgment and decision-making.
Following a personal assessment, our Social Workers gather available resources on the patient's behalf. Plans are made in conjunction with the patient and/or the patient's advocate to offer reasonable options and choices that are compatible with the patient's lifestyle.
The Social Work Department provides education and supportive counseling to patients and their families. Social Workers coordinate patient transfers to a variety of specialized settings, including rehabilitative or skilled nursing facilities, as part of the management of acute, chronic, or terminal illnesses.
For the inpatient and outpatient populations, the department helps access both public and private entitlement programs to enhance the quality of their life, as well as ensure that all of our patients continue to have access to the best available medical care.
Our staff is also committed to supporting patients through the management of chronic medical problems and crisis situations.
